Physician, Home and Supportive Care Specialist

Key Highlights:
- Practice with a multi-disciplinary team of healthcare providers, including Family Practice and/or Internal Medicine Physicians, Nurse Practitioners, and/or Physician Assistants, RN Case Managers, social workers, and clinic staff.
- Our model provides a comprehensive, coordinated approach to care for a predominantly Medicare-aged population.
- Primarily manage care in the patient's home, with occasional office or facility settings depending on patient needs.
- Diagnose and treat acute and chronic conditions, such as diabetes, hypertension, kidney disease, and other chronic medical conditions.
- Treat acute conditions, including minor injuries and other acute medical conditions, such as infections or exacerbations of chronic diseases.
- Facilitate patient and family decision-making regarding treatment decisions and end-of-life care and serious illness discussions.
- Prescribe pharmacological and non-pharmacological interventions.

Compensation and Benefits Highlights:
- Base Salary plus Quality Incentive with a target of $50,000/year.
- Comprehensive benefits, including health, dental, and vision insurance, 401(k) matching, and executive savings plan.
- 40 hours of PTO, 40 hours of CME, and clinics closed for 8 national holidays.

Required Qualifications:
- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.).
- Board certification or eligible in Family Medicine or Internal Medicine.
- Active, unrestricted medical license in Florida.
- Valid DEA license.
- Reliable transportation.
Preferred Qualifications:
- Home health experience.
- Palliative or Hospice experience.
- Bilingual (English/Spanish) proficiency.
