About the Role

We are seeking a highly skilled Speech Language Pathologist to join our Home Care team at AdventHealth Home Care West FL. As a Speech Language Pathologist, you will play a vital role in providing high-quality patient care and improving the lives of our patients and their families.

Key Responsibilities
  • Evaluate patients for appropriateness of home health and develop individualized care plans in consultation with physicians.
  • Provide speech therapy services to patients in their homes, including assessment, treatment, and education.
  • Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal patient outcomes.
  • Document patient progress and communicate with physicians and other healthcare professionals as needed.
  • Participate in performance improvement initiatives and contribute to the development of new programs and services.
Requirements
  • Master's degree in Speech Therapy.
  • Current Speech Therapy License in the State of Practice.
  • Valid Driver's License and current car insurance.
  • BLS certified.
  • Minimum of one year of relevant clinical experience.
Preferred Qualifications
  • Recent experience in a Medicare-certified home health agency as a case manager.
  • Home Health Case-Manager Certification.
  • COS-C Certification.
