Responsibilities:
• Deliver high-quality nursing care in post-acute care facilities
• Observe resident conditions and notify the nursing team of updates
• Monitor and report on resident vital signs
• Ensure a clean and safe environment by performing tasks such as sanitizing surfaces and changing linens
• Assist residents with personal hygiene, feeding, grooming, and other activities of daily living (ADLs)
• Reposition residents and transport them around the facility as needed
• Liaise between staff, residents, and family members to support ongoing communication and transparency
• Engage with residents on a regular basis and respond to their needs
• Be accountable to follow and adhere to care plans for residents, and all applicable laws, regulations, and policies

Qualifications and Skills:
• Current Pennsylvania Certified Nursing Assistant (CNA) certification
• Ability to lift at least 50 lbs
• Knowledge of medical terminology and nursing care procedures
• Strong attention to detail
• Effective communication and social skills
