Sun River Health is seeking a highly skilled Psychiatric Nurse Practitioner to join our team at our Yonkers Park Care location.

Job Summary:

The Psychiatric Nurse Practitioner will provide psychiatric clinical services for patients, including medication management, psych evaluations, and treatment planning. The ideal candidate will have a strong background in mental health care and experience working with diverse patient populations.

Key Responsibilities:
  • Consult with MD/LCSW on patients in the Impact Model, via telephone encounter, ECW consult, and/or with patient visits, as needed to address depression and anxiety
  • Provide medication management for active Sun River Health patients
  • Participate in case conferencing with LCSW/MD and care teams
  • Psych evaluation of pain management patients who are also prescribed psychotropic medications or at the request of their treating physician, in compliance with Sun River Health pain management regulations
  • Approval of treatment plans for patients in BH care at Sun River Health (NYS regulation by an MD)
  • Participate in efforts to support Sun River Health's primary care and BH integration efforts
Requirements:
  • Master's Degree
  • 5-8 years of work-related experience
  • License as a Psychiatric Nurse Practitioner in New York State
  • License to practice medicine in New York State
  • CPR/BLS certified
Preferred Qualifications:
  • Ability to communicate in English/Spanish (orally and written); Spanish preferred
Salary Range:

$29,000 - $31,050 yearly
