Job Title: Psychiatric Nurse Practitioner

Cordial Health is seeking a skilled and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our outpatient psychiatric clinic in Yonkers, NY.

Job Summary

We are looking for a dedicated and passionate PMHNP who is committed to improving the mental health and well-being of individuals in our community. As a PMHNP at Cordial Health, you will be responsible for conducting comprehensive psychiatric evaluations and assessments, diagnosing and developing individualized treatment plans, and providing medication management and counseling services.

Responsibilities
  • Conduct thorough psychiatric evaluations and assessments of patients in an outpatient setting.
  • Develop and implement individualized treatment plans based on patient needs.
  • Provide medication management and adjust treatment plans as necessary.
  • Collaborate with interdisciplinary teams to ensure holistic patient care.
  • Offer counseling and psychotherapy services to patients and their families.
  • Educate patients and families about mental health conditions, treatment options, and self-care strategies.
  • Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
  • Participate in clinical meetings, case conferences, and quality improvement initiatives.
Requirements
  • Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
  • Ability to work independently and effectively manage a caseload of patients.
  • Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
  • Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
  • Proficient in electronic health record (EHR) systems and basic computer skills.
  • Compassionate, patient-centered approach to care.
  • Speaking Spanish is a plus.
