Skilled Nursing Facility Registered Nurse

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Northwell Health. As a Skilled Nursing Facility RN, you will play a vital role in providing exceptional patient care and contributing to the success of our healthcare organization.
Key Responsibilities- Conduct thorough patient interviews, explain policies and procedures, and review patient charts to ensure accurate and courteous care.
- Assess patients' gastrointestinal, cardiovascular, respiratory, renal, and neurological health status, and develop individualized plans of care.
- Communicate and document patients' physiologic health status and plan of care, as well as their psychosocial health status and care plan.
- Develop patient outcome statements and establish individualized patient goals, identifying care activities and establishing priorities to achieve expected outcomes.
- Coordinate the cost-effective use of supplies, equipment, and medication to achieve expected outcomes, and document the plan of care.
- Implement the plan of care, maintaining constant vigilance over patients' care to ensure safety precautions and needs are followed.
- Exercise professional skills related to the plan of care, reassessing patients as needed and revising the plan of care accordingly.
- Administer prescribed treatments, use equipment necessary for patient care, and provide emotional support to patients and their significant others.
- Apply scientific principles in performing procedures, carrying them out safely, timely, and efficiently, and making accurate observations of patients' conditions during treatments and procedures.
- Report and record patients' responses to medical and nursing interventions, and keep accurate documentation of patients' treatment, activity, and condition.
- Participate in planning for discharge and coordinate referrals, as appropriate, and provide discharge instructions to patients and their significant others.
- Evaluate care provided for patient outcomes, demonstrating ability to measure effectiveness of care provided and document same.
- Perform variance analysis related to outcome data for performance improvement, and design, implement, and evaluate systems to improve care in the unit.
- Graduate from an accredited School of Nursing.
- Bachelor's Degree in Nursing, preferred.
- Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
The salary range for this position is $82,500-$136,340/year. Northwell Health is an equal employment opportunity employer and welcomes applications from diverse candidates.
