NYC Health + Hospitals/Gouverneur is a 295-bed Skilled Nursing Facility (SNF) with a 5-Star CMS Quality Rating located in Manhattan's Lower East Side neighborhood. Our facility offers enhanced, cutting-edge healthcare services in a state-of-the-art space, providing 24-hour care for individuals in need of sub-acute care, long-term care, or hospice care.
Job Summary:As a Staff Nurse in our Skilled Nursing Facility, you will provide professional nursing care to patients/clients within an assigned unit. You will diagnose and treat responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling, and the provision of care supportive to and restorative of life and well-being.
Responsibilities:- Plans, implements, and evaluates nursing care utilizing the nursing process.
- Formulates and implements comprehensive and nursing care plan. Sets priorities, delivers, and directs care to meet resident needs.
- Evaluates residents' responses and makes appropriate changes on the care plan, on an ongoing basis. Maintains accurate documentation as described in policies and procedures.
- Collaborates with other members of the interdisciplinary team in the care of residents, including the resident, family, and/or significant other's. Discuss education and discharge planning issues as they arise.
- Transcribes physician's orders and administers medication/treatment as defined in all policies.
- Assumes charge nurse responsibilities as assigned.
- Maintains professional competence through participation in continuing education and other appropriate learning experiences. Attends all mandatory classes and participates in the orientation/training staff.
- Demonstrates safety awareness and follows protocols for fire, disaster, infection control, and other safety practices.
- Participates in performance improvement activities and other committees as assigned.
- Performs other related duties.
- Supervises and directs Licensed Practical Nurse, Medical Surgical Technician, Certified Nurse's Aide, or other allied nursing personnel concerning the care of residents.
- A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State.
- Valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
