Job Description

This Clinical Social Worker position is responsible for providing comprehensive care to patients and their families. The ideal candidate will possess a Master's degree in Social Work from a CSWE accredited program and have experience working in healthcare settings.

Key Responsibilities

  • Conduct thorough assessments and develop treatment plans for patients and their families
  • Provide direct social work services to patients and their families, addressing environmental problems and offering emotional support
  • Collaborate with the care team to ensure continuity of care before and after discharge
  • Maintain accurate records of all activities related to casework, discharge planning, and follow-up
