At Innovative Renal Care, we are seeking a highly skilled Master Social Worker to join our team. As a key member of our dialysis setting, you will play a vital role in helping patients understand their rights and responsibilities, and guiding them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.
Responsibilities:- Assess patients' needs and develop personalized care plans
- Provide education and support to patients and their families
- Collaborate with healthcare teams to ensure comprehensive care
- Meet state regulations and maintain current licensure
- Participate in ongoing education and training to stay up-to-date on best practices
- Master's degree from an accredited school of social work (MSW)
- Current state licensure (e.g. LCSW, LMSW)
- Two years of clinical work experience, preferably in an ESRD setting
- Current CPR certification preferred
