Hospice Case Manager

The Hospice Case Manager is responsible for planning, organizing, and directing hospice care for patients and their families. This role requires a registered nurse with experience in community health education and experience in nursing.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine hospice needs.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide education and support to patients and families on self-care techniques, medication management, and other health-related topics.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are available.
- Current RN licensure in the state of practice.
- Experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
