  • Providing individualized patient care plans and implementing evidence-based practices to promote optimal patient outcomes.
  • Collaborating with interdisciplinary teams to ensure seamless care transitions and effective communication.
  • Participating in quality improvement initiatives to enhance patient satisfaction and clinical outcomes.
  • Developing and maintaining strong relationships with patients, families, and healthcare teams to ensure a positive patient experience.
Responsibilities:

As a Registered Nurse (RN) in our Long Term Acute Care unit, you will be responsible for:

  • Assessing patient needs and developing individualized care plans.
  • Implementing and evaluating the effectiveness of care plans.
  • Providing education and support to patients and families.
  • Collaborating with healthcare teams to ensure effective care transitions.
Requirements:

To be successful in this role, you will need:

  • A valid State RN License.
  • BLS certification at hire.
  • ACLS certification within 6 months of hire.
Preferred Qualifications:

Experience in critical care or long-term acute care is preferred, but not required.
