Aveanna Healthcare is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the comfort of their own homes. Job Summary:We are looking for a skilled LPN to join our team of healthcare professionals who provide high-quality, patient-centered care in the home setting. The successful candidate will have a strong background in nursing, excellent communication skills, and a passion for delivering exceptional patient care.
Responsibilities:- Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse.
- Assess patient needs and develop individualized care plans to meet those needs.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patient progress and report changes to the supervising RN and/or physician.
- Document all patient interactions and care provided in a timely and accurate manner.
- Current, unrestricted LPN license in the state of practice.
- At least 6 months of experience as an LPN in a clinical care setting.
- Current CPR certification.
- Ability to travel to patient homes and work in a variety of environments.
