Job Summary

IntelyCare is seeking a skilled Registered Nurse to join our team of healthcare professionals. As a Registered Nurse, you will provide expert nursing care to patients in post-acute care facilities, working closely with residents and medical staff to develop care plans and ensure the highest level of patient care.

Responsibilities
  • Provide expert nursing care to patients in post-acute care facilities
  • Develop and implement individualized care plans for residents
  • Collaborate with medical staff to ensure the highest level of patient care
  • Monitor and track patient outcomes, making adjustments to care plans as needed
  • Ensure accurate and timely documentation of patient histories and interventions
  • Provide training and supervision to nurses and support staff
  • Participate in incident investigations and quality improvement initiatives
Requirements
  • Current Connecticut state license to practice as a Registered Nurse (RN)
  • Successful completion of an approved nursing program
  • 1 year of experience as a Registered Nurse or in a related role
  • Advanced organizational skills and ability to work independently
