We are seeking a highly skilled and compassionate Case Manager to join our team at Pediatric Home Healthcare. As a key member of our staff, you will be responsible for ensuring the highest quality of patient home care through direct case management.
Key Responsibilities- Perform and document pediatric patient assessments to ensure thorough and accurate documentation.
- Ensure that documentation submitted to insurance is thoroughly completed and compliant with regulatory standards.
- Conduct Skills Reviews for field nurse applicants to ensure they meet our high standards.
- Collaborate with the Director of Patient Care Services to monitor QAPI program and identify problem areas.
- Promote and educate regarding infection control and standard precautions to prevent contamination and transmission of disease.
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Minimum 2 years' experience in pediatric, NICU, or home healthcare nursing.
- Minimum 1 year's experience in pediatric home healthcare is strongly preferred.
- Current Healthcare Provider BLS from American Red Cross or American Heart Association (or subsidiary).
- Strong English language skills; bilingual in Spanish preferred.
