Job Summary:

As an Ambulatory Care Pharmacist at Kaiser Permanente, you will provide high-quality, evidence-based pharmaceutical care services to patients in a collaborative environment. You will work closely with other healthcare professionals to manage patient drug therapy, provide patient education, and prepare and/or oversee the preparation and dispensing of medications.

Key Responsibilities:

  • Initiate, adjust, and monitor drug therapy based on patient information and medical records.
  • Conduct patient medication history inquiries and provide education on disease states, medication use, and potential side effects.
  • Collaborate with healthcare teams to optimize patient outcomes, reduce medication errors, and improve patient compliance.
  • Perform drug utilization review, comprehensive medication review, and medication reconciliation.
  • Provide education to providers and staff on pharmaceutical care and medication management.

Requirements:

  • Pharmacist License (California) required.
  • Minimum 12 months experience in ambulatory care pharmacy services.
  • Knowledge of current pharmacological principles, medical terminology, and disease state management.

Preferred Qualifications:

  • Doctorate of Pharmacy, Pharm.D.
  • PGY-2 clinical pharmacy specialty residency or fellowship.
  • Board certification in relevant practice area.
