Key Responsibilities
  • Develop and implement individualized care plans for pediatric patients in their own homes, utilizing the nursing process to ensure high-quality patient outcomes.
  • Collaborate with healthcare professionals and families to provide comprehensive care, including medication management, wound care, and education on disease management and prevention.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies to address them.
  • Provide emotional support and education to patients and their families, empowering them to take an active role in their care.
  • Document patient progress and outcomes, maintaining accurate and detailed records of care provided.
Requirements
  • Current Registered Nurse (RN) license for the state in which you practice.
  • Current Health Certificate, as required by facility or state regulation.
  • Current PPD or Chest X-Ray, as required by facility or state regulation.
  • Current BLS card.
  • One year prior Registered Nurse (RN) experience, preferred.
