About the Role

We are seeking a skilled RN Infusion Liaison to join our team at Providence Home Services Oregon. As a key member of our infusion nursing team, you will provide direct nursing services related to infusion therapy for patients in the hospital, home, clinic, or alternate living site.

Key Responsibilities
  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Deliver infusion nursing care in the home or clinic, prioritizing patient safety and satisfaction.
  • Develop and implement individualized care plans, incorporating patient preferences and needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care.
Requirements
  • Current Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Licensure in states where care is delivered upon hire.
  • Education to meet certification, license, or registration requirements.
Preferred Qualifications
  • 1 year of experience in acute hospital care, infusion, or home health.
