We are seeking a skilled RN Infusion Liaison to join our team at Providence Home Services Oregon. As a key member of our infusion services team, you will play a vital role in providing direct nursing services to patients in various settings, including hospitals, homes, clinics, and alternate living sites.
Key Responsibilities- Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
- Offer infusion nursing care in the home or clinic, collaborating with various providers, patients, caregivers, physicians, and payors.
- Ensure ongoing care through coordination and collaboration with the patient, caregiver, physician, and payor.
- Education to meet certification, license, or registration requirements.
- Oregon Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- Must be licensed in states wherever care is delivered upon hire.
- 1 year of experience with acute hospital care, infusion, or home health.
