Job Summary
The Hospice Nurse Practitioner will work closely with our Interdisciplinary Group (IDG) to ensure the best possible care for our patients. This includes collaborating with the IDG to evaluate and treat terminal disease, educating patients and families about disease processes and self-care techniques, and providing training to other staff members as needed. The ideal candidate will have excellent communication skills, be able to work in an unstructured environment, and be knowledgeable about hospice criteria and palliative medicine.
Responsibilities
Collaborate with the IDG to ensure appropriate evaluation and treatment of terminal disease
Educate patients and families about disease processes and self-care techniques
Provide training to other staff members as needed
Perform face-to-face visits at patients' places of residence
Interact with attending and consulting physicians as necessary
Maintain up-to-date knowledge in pharmacology and palliative medicine
Meet hospice criteria requirements
Requirements
Current STATE RN and Advanced Practice Nurse License
Valid Driver's License
Valid Auto Insurance
AHA CPR Certification
HomeCare HomeBase experience preferred
