About the Role

Beebe Healthcare is seeking a highly skilled Clinical Resource Pharmacist to join our team in a decentralized practice setting. As a key member of our interdisciplinary team, you will work closely with physicians, nurses, and other healthcare professionals to ensure the best possible health outcomes for our patients.

Key Responsibilities
  • Provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting.
  • Support centralized and decentralized medication-use systems, and deliver optimal medication therapy to patients with a broad range of disease states.
  • Participate in all necessary aspects of the medication-use system, including assessing patient needs, incorporating age and disease-specific characteristics into drug therapy and patient education, and adjusting care according to patient response.
  • Provide clinical interventions to detect, mitigate, and prevent medication adverse events.
  • Collaborate with healthcare providers within decentralized patient care areas to provide patient-centered care.
  • Identify high-risk patients and implement interventions to improve quality and safety.
  • Make appropriate evidence-based, patient-centered medication recommendations.
  • Participate in the management of medical emergencies.
  • Provide discharge medication review, reconciliation, and counseling as appropriate.
  • Act as a drug information resource for clinical staff within the organization.
  • Comply with hospital and pharmacy policies and procedures.
  • Assist with creating/maintaining hospital order sets, guidelines, and standard operating procedures.
  • Ensure appropriate drug utilization and perform drug utilization reviews.
  • Support the initiatives of teaching and learning, promoting the growth and education of team members and students.
Requirements
  • Doctor of Pharmacy (PharmD) degree.
  • Pharmacist licensure in the state of Delaware.
  • Clear communication skills, both written and verbal.
  • Able to keep confidential information regarding patients and team members.
  • Able to withstand crisis situations.
  • Has skills to provide customer service to patients, team members, and visitors.
