Key Responsibilities- Provide direct patient-centered care and integrated pharmacy operational services in a decentralized practice setting.
- Support centralized and decentralized medication-use systems, ensuring seamless medication therapy management.
- Participate in quality management programs to monitor processes, analyze data, and implement interventions to improve services.
- Collaborate with healthcare providers to identify high-risk patients and implement interventions to improve quality and safety.
- Make evidence-based, patient-centered medication recommendations and provide discharge medication review, reconciliation, and counseling as needed.
- Doctor of Pharmacy (PharmD) degree.
- Pharmacist licensure in the state of Delaware.
- Clear communication skills, both written and verbal.
- Able to keep confidential information regarding patients and team members.
- Able to withstand crisis situations.
