Job Summary:
Clinical Management Consultants is seeking a highly skilled and experienced Nurse Director - Case Management to lead our case management department. As a key member of our leadership team, you will be responsible for overseeing the operations of the department, including LCSW Social Work staff and RN Case Managers.
Key Responsibilities:
- Oversee hospital case management department operations, including LCSW Social Work staff and RN Case Managers
- Identify and initiate performance improvement activities to enhance quality patient care and safety
- Develop and implement strategies to improve patient outcomes and satisfaction
- Collaborate with other departments to ensure seamless care coordination
- Provide leadership and guidance to team members to ensure they have the necessary resources and support to perform their jobs effectively
Requirements:
- Current RN licensure and a minimum of 5 years of experience in case management
- Proven leadership and management skills, with experience in supervising and developing staff
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Ability to analyze data and make informed decisions to drive quality improvement initiatives
- Knowledge of case management principles and practices, with a focus on patient-centered care
