Position Summary:
This role is funded through grants. The Housing Support Specialist is tasked with delivering extensive case management services aimed at assisting individuals facing chronic homelessness in their journey towards secure housing. This position emphasizes collaboration with outreach teams, landlord engagement, and maintenance teams to support clients in transitional settings and provide continued assistance for a year post-housing.
Key Responsibilities:
Case Management and Client Support:
1. Oversee a caseload of up to 20 households experiencing chronic homelessness, offering thorough support and guidance throughout their transition to stable housing.
2. Work in partnership with outreach teams, landlord engagement, and maintenance teams to assist individuals in transitional settings and ensure successful housing placements.
3. Aid clients in completing housing applications, vouchers, and other necessary documentation promptly to facilitate housing placements.
4. Advocate for clients' rights and preferences, ensuring their autonomy and dignity are respected throughout the case management process.
5. Coordinate with housing providers and landlords to ensure smooth transitions for clients moving into their new homes.
6. Conduct comprehensive assessments of clients' needs and goals to create tailored service plans.
7. Provide one year of weekly, in-person case management services in clients' residences, covering intake, long-term housing stabilization, budget planning, and connections to health services.
8. Regularly assess and monitor clients' progress towards housing stability, adjusting service plans as necessary.
9. Assist clients in building support networks that promote independence and self-sufficiency, connecting them with community resources.
10. Educate clients on relevant topics such as community safety and healthy relationships to promote informed decision-making.
11. Offer transportation assistance to clients for appointments and services as needed.
Documentation and Reporting:
1. Ensure timely and accurate program enrollments in the Homeless Management Information System (HMIS).
2. Maintain detailed and accurate case documentation, including all client interactions.
3. Comply with the latest Coordinated Entry Policies and Continuum of Care (CoC) Standards of Care.
Collaboration and Community Involvement:
1. Partner with other service providers to enhance clients' access to supportive resources.
2. Act as a liaison for Coordinated Entry staff and behavioral health partners to ensure effective service coordination.
3. Participate in community meetings to share updates and collaborate on strategies to support individuals experiencing homelessness.
4. Engage actively in community efforts to address homelessness.
5. Approach all interactions with compassion and cultural sensitivity.
Qualifications:
- Experience working with individuals facing chronic homelessness, including outreach and case management.
- Knowledge of community resources for mental health, employment, and healthcare.
- Strong communication skills to build rapport with clients and stakeholders.
- Ability to work independently and manage documentation effectively.
- Proficient in using computers and data management systems.
- Skills in assessing client needs and developing individualized plans.
- Empathetic and patient approach when engaging with clients.
- Excellent organizational skills for managing caseloads and administrative tasks.
- Valid driver's license and reliable transportation.
- Ability to pass background checks.
- Commitment to positively impacting the lives of individuals experiencing homelessness.

Grant-Funded Position Disclaimer:
This position is contingent upon grant funding availability. Employment is at-will, meaning either the employer or employee can terminate the relationship at any time.
