Responsibilities:
  • Coordinates case management activities within an interdisciplinary health care team to ensure appropriate delivery of services to patients.
  • Focuses on customer service and continually strives to perform the duties of their job in a manner that will result in optimal patient care.
  • Initiates, coordinates and ensures ongoing communication with the physician and other members of the interdisciplinary health care team.
  • Provides direct patient care, evaluates outcomes, and adjusts nursing care process as indicated to ensure optimal patient care.
  • Establishes the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with the physician orders and patient needs.
  • Assigns other interdisciplinary team members as appropriate to assist in the appropriate delivery of services as ordered on the plan of care to the patient.
  • Attends and participates in case conferences, team meetings, and other work-related meetings.
  • Recognizes limitations and requests assistance and direction to meet performance goals.
  • Completes all necessary documentation which may include visit assessment, plan of treatment, and verbal orders accurately and promptly in electronic documentation system while in the patient's home and within the agency's and regulatory standards.
Requirements:
  • Current licensure as a Registered Professional Nurse either in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
  • Graduate nurses must complete licensure examination within SIX (6) MONTHS of hire or within one year of graduation, whichever comes first.
