Responsibilities- Coordinate case management activities within an interdisciplinary healthcare team to ensure appropriate delivery of services.
- Initiate, coordinate, and ensure ongoing communication with the physician and other members of the healthcare team.
- Provide direct patient care, evaluate outcomes, and adjust the nursing care process as indicated to ensure optimal patient care.
- Establish the plan of care in collaboration with the physician, patient, and interdisciplinary team members in accordance with physician orders and patient needs.
- Assign other interdisciplinary team members as appropriate to assist in the delivery of services as ordered on the plan of care.
- Teach other nursing personnel, including LPNs and Home Health Aides, as needed.
- Attend and participate in case conferences, team meetings, and other work-related meetings.
- Recognize limitations and request assistance and direction to meet performance goals.
- Complete all necessary documentation, including visit assessment, plan of treatment, and verbal orders, accurately and promptly in the electronic documentation system.
- Current licensure as a Registered Professional Nurse in the state where the facility is located or in a state covered by a licensure compact agreement with the state where the facility is located.
- CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.
- Graduate nurses must complete licensure examination within six months of hire or within one year of graduation, whichever comes first.
- Zero to two years of experience preferred, with six months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred.
- BSN preferred.
- Ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families.
- Ability to work productively and effectively within a complex environment, handle multiple/changing priorities, and specialized equipment.
