Comunilife is seeking an experienced Substance Abuse Counselor to join its team in Brooklyn, NY. As a member of the Comprehensive Case Management (CCM) Team, the successful candidate will conduct screenings and assessments, develop and implement person-centered treatment plans, and provide supportive services for formerly homeless residents in the Safer Haven program.

Job Summary:
  • Conduct screenings and assessments to identify interrelated issues of substance abuse, health, behavioral health, and other care needs.
  • Develop and implement person-centered treatment plans, and provide supportive services for clients.
  • Deliver evidence-based client treatment practices, including Individualized Dual Diagnosis Treatment (IDDT), the Multicultural Relational Approach, Wellness Self-Management, Motivational Interviewing, and Individualized Wellness Plans.
  • Assess clients to determine appropriate internal and external referrals.
  • Provide individual, family, and group counseling, as well as psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems.
  • Engage in discharge planning and linking clients with at least 90 days of post-discharge services.
  • Maintain client case records and write client documentation and program narrative and/or statistical reports as needed.
Requirements:
  • Bachelor's Degree in a human services field, such as social work, psychology, etc.
  • At least three years of prior experience working with clients with substance abuse disorders.
  • CASAC certification (trainees will not be considered).
  • Experience working with formerly homeless residents who are seriously mentally ill with co-occurring substance abuse disorders and chronic health conditions is preferred.
  • Excellent written and oral communication skills; computer literacy.
  • Bilingual (English/Spanish) a plus.
