Job SummaryWe are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessor. As a key member of our clinical team, you will conduct comprehensive health assessments, medical history reviews, and provide health education to patients in their homes.
Key Responsibilities:- Conduct adult/geriatric assessments, medical history reviews, and provide health education to patients in their homes
- Collaborate with physicians, case managers, social workers, and other healthcare professionals to ensure comprehensive care
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits
- Work collaboratively with primary care physicians to provide patient education and follow-up care
- Master's degree in nursing or related field
- Current RN and NP licensure in the state of practice, including prescription authority
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health record software
- Excellent verbal and written communication skills

This is a PRN/Part Time position, with flexible scheduling and the opportunity to work in a variety of settings, including home visits, skilled facility visits, and telehealth visits. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
