Job Title: Wound Care Nurse Practitioner

Theoria Medical is seeking a dedicated and skilled Wound Care Nurse Practitioner to join our team. As a Wound Care Nurse Practitioner, you will be responsible for providing high-quality, patient-centered wound care services to patients residing in skilled nursing facilities (SNFs).

Job Summary:

We are looking for a highly motivated and experienced Wound Care Nurse Practitioner to join our team. The successful candidate will have a strong background in wound care and a passion for delivering exceptional patient care. As a Wound Care Nurse Practitioner, you will work closely with our wound care team to assess and treat patients with wounds, providing comprehensive wound care services and collaborating with facility staff to ensure seamless care delivery.

Responsibilities:
  • Provide high-quality, patient-centered wound care services to patients residing in SNFs
  • Collaborate with wound care nursing staff to assess and treat patients with wounds
  • Document patient care and encounters in a timely and accurate manner
  • Participate in wound rounds with facility staff to ensure comprehensive care delivery
  • Provide education and training to facility staff on wound care best practices
Requirements:
  • Valid PA/NP license in the state of practice
  • Active DEA license
  • Valid NP/PA degree from an accredited institution
  • General post-acute/SNF knowledge or experience preferred
  • Familiarity with EMR systems and charting practices
