Nurse Practitioner

We are seeking a highly skilled Nurse Practitioner to join our team at Optum Home & Community Care. As a key member of our HouseCalls product, you will play a vital role in providing annual clinical assessments to patients in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform members' primary care physicians of potential gaps in care.
- Intervene in urgent and emergent situations as needed.
- Educate members on disease processes, medications, and compliance.
- Active NP license or ability to obtain by start date, with unencumbered licensure.
- APCs must have obtained approval to practice autonomously or without formal supervision, if applicable.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or ability to obtain by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
- Reliable transportation for home visit assessments.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, up to 5% of the time, with advanced notice.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages.
