Home-Based Medical Care Specialist

**Job Summary:**
We are seeking a skilled Physician Assistant to join our team in New London County, CT.
The successful candidate will work as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care of a population within an assigned geography.
This role is ideal for someone who is passionate about improving the health and well-being of patients, particularly those with complex chronic conditions.
Key Responsibilities:- Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care thereafter
- Develop patient management care plans upon enrollment and update them as needed
- Meet with patients and their legal representatives to review care plans
- Prescribe diagnostics and interventions to avoid unnecessary acute admissions
- Complete follow-up and post-discharge assessments
- Educate patients on disease processes and ways to manage them effectively
**Requirements:**
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN/NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
**Salary and Benefits:**
A competitive salary of $88,000 to $173,200 annually, depending on experience, plus a comprehensive benefits package, incentive programs, and opportunities for professional growth and development.
