Job Overview

The Advanced Practice Nurse (specializing in family medicine, pediatrics, or psychiatry) delivers exceptional healthcare services within their specific area of expertise, focusing on health promotion and disease prevention. This role involves practicing autonomously while collaborating with a designated licensed physician who serves as the primary collaborating physician. Responsibilities and performance metrics are defined through the establishment of annual objectives and productivity benchmarks.

Key Responsibilities:

  • Evaluates primary healthcare needs by gathering comprehensive health histories, conducting physical assessments, ordering and/or performing relevant diagnostic tests, analyzing collected data to ascertain patient health status, and developing a problem list.
  • Delivers coordinated and ongoing primary healthcare, pediatric care, or psychiatric care (as appropriate based on training and privileges) to patients, which includes collaborative goal setting and care delivery planning with the patient, their family/caregiver, and community treatment teams when necessary.
  • Prescribes a range of treatment options to address the health and illness requirements of patients.
  • Maintains comprehensive, accurate, and continuous documentation in patients' electronic health records in accordance with organizational standards.
  • Oversees and assesses the care plan process and outcomes, making modifications as necessary.
  • Works in partnership with other multidisciplinary professionals and community organizations to ensure high-quality, age-appropriate, and culturally-sensitive care.
  • Coordinates services with team members, leads daily huddles, and responds to electronic alerts to meet patient care needs and ensure continuity of care for a panel of up to 1,200 unique patients annually.
  • Engages in quality assurance initiatives through collaborative peer review, development of practice protocols, and identification of quality care indicators; contributes to the creation of joint practice protocols and evaluates clinical care standards based on the quality assurance framework.
  • Takes responsibility for maintaining clinical competence by fostering a mentorship relationship with a designated physician; actively pursues professional development by attending provider meetings, utilizing electronic resources such as Up to Date, acquiring in-depth knowledge of policies and procedures, and participating in continuing education necessary for clinical licensing and board certifications.
  • Maintains productivity levels that align with experience, specific practice settings, and annual objectives.
  • Operates within the legal and ethical frameworks established by the American Medical Association, the State of Illinois Department of Professional Regulation, and Specialty Standards of Practice (e.g., pediatrics, family practice, psychiatry).
  • Supervises advanced practice nursing students as appropriate through formal agreements with educational institutions.

Qualifications:

  • Possession of an active Illinois Registered Nurse license. A Bachelor's degree in nursing or a master's or doctorate in advanced practice nursing from an accredited institution with a nurse practitioner specialty is required. Certification in the specialty area must be documented. One to three years of experience as a nurse practitioner is preferred.
  • Additional documented clinical experience, education, or certification in a related field may be necessary.
  • Proficiency in Spanish and/or other languages is highly desirable.
