Job Summary

Gateway CSB PEO LLC is seeking a highly skilled and compassionate Case Manager to join our team. As a key member of our interdisciplinary team, the successful candidate will provide ongoing supportive and case management functions to consumers with long-term mental illness.

Key Responsibilities
  • Documentation and Compliance
    • Accurately record services that relate directly to treatment outcomes within approved timeframes.
    • Complete required clinical documentation according to agency standards.
    • Maintain all documentation in accordance with applicable policies, laws, and instructions.
    • Ensure that all services provided are within guidelines and document care in compliance with agency requirements and standards.
    • Maintain a minimum chart audit score of 70% or better for all consumers on caseload.
    • Ensure all weekly reports have been addressed and cleaned up (if necessary) within hours specified by supervisor.
  • Community Outreach
    • Collaborate with behavioral health providers, hospitals, emergency rooms, jails, and shelters through regular meetings to engage and transition consumers throughout systems of inpatient and community care.
  • Productivity
    • Meet minimum direct time requirements of individual billed hours per year.
    • Enroll no fewer than 5% individuals into case management services each month until reaching capacity.
    • Deliver a minimum of 2 face-to-face visits on a monthly basis.
Requirements
  • Bachelor's degree from an accredited program in a helping field.
  • Minimum of 2 years related experience.
  • Valid GA Driver's license.
Work Environment

This job operates in a professional office environment. The employee is frequently required to walk, sit, stand, or kneel and occasionally required to climb or balance and stoop. The employee must frequently lift and/or move up to 15 pounds.
