Clinical Nutrition Specialist

At Trinity Health, we are seeking a highly skilled Clinical Nutrition Specialist to join our team at Mt. Sinai Rehab Hospital.
About the Role:
This is a full-time, 40 hour per week position working day shifts.
Job Summary:
The Clinical Nutrition Specialist will provide optimal nutritional care to patients by assessing their needs, designing care plans, and monitoring progress.
Duties and Responsibilities:
- Nutrition Assessments: Conduct comprehensive nutrition assessments of patients using established guidelines.
- Care Plan Development: Create individualized care plans that address patients' nutritional needs and goals.
- Patient Education: Provide education on modified diets, medical nutrition therapy, and other relevant topics.
- Nutrition Support Management: Assist in managing enteral and parenteral nutrition as needed.
- Documentation: Accurately document all assessments, care plans, and patient interactions in the electronic medical record.
- Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care.
- Training and Development: Participate in training and educating nutrition interns.
Requirements:
- Bachelor's degree in food and nutrition, dietetics, or a related field.
- Registration as a dietitian with the Commission on Dietetic Registration (CDR) and state licensure as a licensed dietitian/nutritionist (LDN).
- Minimum one year of experience as a dietitian, preferably in a healthcare setting.
- Analytical and communication skills.
