Mental Health Resource Center Inc is seeking a highly skilled and compassionate Suicide Prevention Care Coordinator to join our team. As a key member of our Suicide Prevention team, you will work with individuals who have been hospitalized for suicidal attempts or suicidal ideation, assessing their needs and coordinating a plan for rapid follow-up after discharge from emergency departments and inpatient psychiatric facilities.
Key Responsibilities- Conduct initial and ongoing assessments to identify individual needs, including suicide assessments, substance abuse, living situation, support system, mental status, history, strengths, and barriers.
- Develop and implement care plans in collaboration with individuals, family members, service providers, and significant others, utilizing individual strengths and addressing identified needs.
- Provide comprehensive risk assessments and biopsychosocial assessments on eligible participants, creating a safety plan and providing lethal means counseling.
- Make assertive contacts with enrolled participants based on suicide risks, providing face-to-face contacts within 24 hours of missed appointments and coordinating a transition of care plan.
- Monitor and document progress or lack of progress for individuals, advocating for acquisition of necessary services and resources.
- Bachelor's Degree in Social Work or a related Human Services field from an accredited university or college.
- Two years of experience working in human services or a mental health-related field, with experience working with adult individuals with mental illness preferred.
- Proficiency in the RBHS/MHRC Electronic Health Records (EHR) and Patient Information System, as well as Microsoft Office, Outlook, and the Internet.
- Valid Florida driver's license and insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits.
