Job Title: Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Chenega Government Mission Solutions, LLC. As an LPN, you will play a vital role in providing exceptional patient care and support to our clients.

Job Summary:

The LPN will be responsible for providing direct patient care, vital signs, scheduling, and greeting patients and visitors. You will also be responsible for screening for health measures, obtaining medication from the pharmacy, and administering medication to patients within your scope of practice.

Responsibilities:
  • Perform direct patient care, vital signs, scheduling, and greeting patients and visitors
  • Screen for health measures, obtain medication from the pharmacy, and administer medication to patients within your scope of practice
  • Immunizations for pediatric and adult patients, preparing the patient for the provider visit, and managing the provider's scheduled patient load on a daily basis
  • Obtain medical records for appointment prior to patients' arrival, check the charts for completeness, and place the necessary forms in the chart
  • Ensure that a consent for treatment has been obtained from the patient prior to any invasive procedure
  • Interview patients and obtain sufficient information necessary to determine primary need, provide assistance or make referrals if necessary to appropriate personnel when the matter is outside of your scope of assignment
  • Assure that patients are properly checked/logged into the clinic and patient forms are initiated using the automated system utilized by the facility
  • Follow standing orders to provide electronic order entry for patient visit, direct patients to testing areas, i.e., laboratory, x-ray, etc., as previously ordered by the physician
  • Review documents such as consultations, referrals, and reports and forward to the appropriate physician or nurse
  • Monitor the clinic flow of patients for those who become increasingly ill, agitated, or who have been waiting for a long period of time
  • Check patients out in EHR/RPMS, ensure contact information is obtained when needed, and that there is a Release of Medical Information form for patients being referred to outside facilities as per protocol
  • Route correspondence appropriately to the patient via mail, fax, computer according to HIPPA regulations in regard to pre-appointment letters, cancellations, and no-show letters
  • Arrange for various clinic appointments utilizing the EHR/RPMS/GUI appointment system
  • Assist patients in contacting patient care services, i.e., Benefits Coordinator, Contract Health, etc.
  • Coordinate clinic scheduling, ensure that appropriate time slots are provided for each of the specialty clinics
  • Provision of all nursing duties as provided for in the LPN scope of practice
  • Participate in clinic staff meetings, in-service, and educational programs
  • Provide proper and adequate services for patient care in accordance with standard of nursing practices and shall be equal in quality to services normally performed for the general public in a Joint Commission of Accreditation for Healthcare Organization accredited facility
  • Floating to other departments as needed
Qualifications:
  • Practical Nursing Diploma from a Vo-Tech school
  • Possess a current, valid, unrestricted LPN license within the Continental United States throughout the term of this contract
  • Possess a valid state driver's license throughout the duration of this contract
  • Physical ability to perform assigned tasks, able to lift fifty (50) pounds, and able to stand and walk for extended periods of time
Knowledge, Skills, and Abilities:
  • Monday through Friday, occasionally some weekends for the Health Centers and Sunday-Saturday 8 and 12 hour day and/or night shifts for the Hospitals
  • Proficient in documentation, entering data, utilizing email, and completing all required training
  • Excellent written and oral communication skills
