We are seeking an experienced Hospice Licensed Master Social Worker (LMSW) to join our team, providing emotional, social, and practical support to patients and families navigating the hospice journey.

Your Role

As a Hospice LMSW, you will work closely with our interdisciplinary team to develop personalized care plans tailored to each patient's unique needs. Your key responsibilities will include:

  • Patient Assessment: Conduct comprehensive evaluations to identify psychosocial, financial, and environmental needs of hospice patients.
  • Plan of Care Development: Collaborate with the care team to create and implement care plans that address the patient's specific needs.
  • Support & Counseling: Provide counseling and guidance to patients and families, addressing emotional and practical concerns, and facilitating coping strategies.
  • Communication: Share significant findings and updates with physicians, supervisors, and care teams, ensuring seamless care coordination.
  • Safety Advocacy: Identify and address unsafe conditions, ensuring the safety and well-being of patients in their care settings.
  • Education: Empower patients and families by providing education about available resources, coping mechanisms, and care expectations.

Requirements

To be successful in this role, you will need:

  • Master's or Doctoral Degree in Social Work from an accredited program by the Council of Social Work Education.
  • At least 1 year of experience in a healthcare setting, preferably in hospice or home health.
  • Current state license as a Social Worker (LMSW).
  • Strong verbal and written communication abilities, with a compassionate and patient-focused mindset.
