Job Summary:
Clinical Management Consultants is seeking a dedicated RN Case Manager Supervisor to oversee the daily operations of our Case Management department. As a key member of our team, you will be responsible for conducting Open Chart Reviews, leading new employee orientation and ongoing training, and supporting Case Managers by escalating complex cases.
Key Responsibilities:
- Conduct Open Chart Reviews to ensure accurate and timely documentation
- Lead new employee orientation and ongoing training to ensure Case Managers are equipped with the necessary skills and knowledge
- Support Case Managers by escalating complex cases and providing guidance and resources as needed
- Participate in performance improvement activities to drive quality and efficiency
- Present at hospital-wide Performance Improvement Meetings to share best practices and lessons learned
Requirements:
- Bachelor's Degree in Nursing (Master's Degree preferred)
- New York RN licensure with current registration
- Patient Review Instrument Assessor Certification
- CCM certification a plus
- At least 5 years of experience as a Case Manager, with Med/Surg experience required and Critical Care and Disease Management experience preferred
- Knowledge of case management practices, nursing principles, regulatory compliance, and Allscript Care Management
- Strong analytical, problem-solving, and communication skills
- Basic computer skills and effective leadership abilities
