Key Responsibilities:

  • Conduct Open Chart Reviews to ensure accurate and timely documentation
  • Lead new employee orientation and ongoing training to ensure Case Managers are equipped with the necessary skills and knowledge
  • Support Case Managers by escalating complex cases and providing guidance and resources as needed
  • Participate in performance improvement activities to drive quality and efficiency
  • Present at hospital-wide Performance Improvement Meetings to share best practices and lessons learned

Requirements:

  • Bachelor's Degree in Nursing (Master's Degree preferred)
  • New York RN licensure with current registration
  • Patient Review Instrument Assessor Certification
  • CCM certification a plus
  • At least 5 years of experience as a Case Manager, with Med/Surg experience required and Critical Care and Disease Management experience preferred
  • Knowledge of case management practices, nursing principles, regulatory compliance, and Allscript Care Management
  • Strong analytical, problem-solving, and communication skills
  • Basic computer skills and effective leadership abilities
