Key Responsibilities1. Facilitate essential connections, referrals, and follow-up for clients to various services and support systems, including long-term care options.
2. Engage with both internal and external service providers to ensure alignment between client needs and treatment objectives.
3. Coordinate healthcare appointment scheduling for clients across mental health, substance use, and physical health services.
4. Ensure clients have reliable transportation to and from appointments, providing direct transport when necessary.
5. Participate in discharge planning processes and maintain communication with other healthcare providers and family members.
6. Collaborate with emergency department personnel to ensure seamless patient care and treatment coordination.
7. Assist clients in acquiring necessary medications and enhancing adherence to prescribed treatment regimens.
8. Work in partnership with community organizations to effectively coordinate client services.
9. Advocate for clients who may require additional support or are in vulnerable situations.
10. Complete and submit all required documentation promptly and accurately.
11. For clients funded through grants, ensure completion of all necessary documentation as per grant requirements.
12. Connect clients with available recovery support resources.
13. Ensure that all necessary releases of information and documentation are completed and properly filed.
14. Contribute to Quality Improvement initiatives.
15. Treat all individuals with respect and dignity while upholding the agency's standards of confidentiality and professionalism.
16. Adhere to all HIPAA and confidentiality regulations as outlined in agency policies.
17. Comply with all agency policies, procedures, and regulatory standards.
18. Perform additional duties as assigned.
Qualifications and Skills1. A Bachelor's degree in a human services discipline or a minimum of three years of relevant experience working with individuals diagnosed with co-occurring disorders.
2. Commitment to the mission of Prairie Ridge Integrated Behavioral Healthcare and the populations served.
3. Strong oral and written communication skills, along with proficiency in documentation practices.
4. Technological proficiency, particularly in the use of electronic health records.
5. Willingness to travel as required.
6. Physical requirements include the ability to sit, use hands for various tasks, and communicate effectively. Frequent walking, stooping, kneeling, and occasional lifting of up to 35 pounds may be necessary.
7. Vision capabilities should include close vision, distance vision, color vision, and the ability to adjust focus as needed.
