Job Summary:

Atrium Health Charlotte is seeking a skilled Healthcare Educator to join their team in Charlotte, North Carolina.

The successful candidate will be responsible for supporting the Outpatient and Community Care Management Program through education, guidance, and mentoring. This role requires expertise in case management, discharge planning, and disease management principles.

We offer a competitive salary of $83,000 per year, based on industry standards for similar positions in the Charlotte area.

Responsibilities:
  • Assist with the development and implementation of educational programs for healthcare professionals.
  • Serve as a resource for leaders and teammates, providing performance metrics and process improvement support.
  • Collaborate with the multidisciplinary care team to achieve timely, appropriate patient management.
  • Develop and maintain accurate case records, documenting fully and accurately in computer databases and documentation systems.
Requirements:
  • BSN required; Masters preferred.
  • Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside.
  • At least two years' experience required in healthcare, including case management/discharge planning in one of the following settings: Acute care, home care, LTC care, Physician Office, or Managed Care company.
  • Care Management certification (CCM, ACM, ANCC) required.
