Job Description:

  • Manage patient care plans and coordinate services with healthcare providers.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.

Required Skills and Qualifications:

  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Valid CPR certification.
  • Licensed driver with an automobile that is insured in accordance with state and/or organization requirements.
