Patient-Centered Care Specialist - Psychiatric Mental Health

Job Summary:

The Stony Brook WTC Health Program is seeking a highly skilled Patient-Centered Care Specialist - Psychiatric Mental Health to join our team. As a key member of our healthcare team, you will provide comprehensive, integrative care to patients with 9/11-related illnesses. Your expertise in psychiatric mental health nursing will enable you to conduct monitoring examinations, provide psychiatric assessments, and develop treatment plans to address the complex needs of our patients.

Responsibilities:
  • Conduct monitoring examinations for WTC Monitoring Program patients under the supervision of the Director or designee.
  • Provide psychiatric assessments, assist with making diagnoses, and develop referral recommendations.
  • Prescribe psychiatric medications as needed and provide treatment to WTC Program patients for WTC-covered conditions.
  • Actively participate in our Collaborative Care Model of healthcare delivery.
  • Make referrals as appropriate to other healthcare providers and/or consult with physicians for further definitive therapy.
  • Perform necessary administrative duties associated with the WTC program, including completing final letters, Workers Compensation, SSI, LODI, and attorney-requested forms.
  • Work with Cerner electronic medical records system for all patient charting documentation.
  • Maintain ongoing communication with physicians, social workers, Clinical Administrator, nurses, and other staff relative to select aspects of patient care or administrative issues.
Requirements:
  • A license to practice as a Registered Nurse in New York State.
  • Masters degree (foreign equivalent or higher) in Nursing.
  • Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in the state of practice, as recognized by the American Nurses Credentialing Center Adult PMHNP certification.
  • One (1) year of clinical experience providing psychiatric services.
Preferred Qualifications:
  • Doctor of Nursing Practice - DNP (or foreign equivalent).
  • Two (2) years of clinical experience in an outpatient program providing psychiatric services in a primary care setting.
  • One (1) year of full-time work experience as a Nurse Practitioner in general internal medicine.
  • Experience with mood and anxiety disorders, including PTSD and cognitive impairments.
  • Prior experience with identification and treatment of survivors of trauma.
  • Familiarity with NCCN guidelines and performing phlebotomy, conducting spirometry tests, and EKGs.
  • Knowledge of the WTC Health Program.
Work Environment:

The incumbent must be willing to work and travel between the Westbury and Commack clinic locations. Occasional evenings and Saturdays will be required.
