Job Title: Per Diem Nurse - Intermediate Care

We are seeking a skilled and compassionate Per Diem Nurse to join our team in Intermediate Care. As a Per Diem Nurse, you will have the opportunity to work in a dynamic and fast-paced environment, providing high-quality patient care and making a meaningful difference in the lives of our patients.

Job Summary:

The Per Diem Nurse - Intermediate Care will be responsible for providing direct patient care to patients in the Intermediate Care unit. This includes assessing patients' physical and emotional needs, developing and implementing individualized care plans, and collaborating with the interdisciplinary team to ensure seamless care transitions.

Responsibilities:
  • Provide direct patient care to patients in the Intermediate Care unit, including assessing patients' physical and emotional needs, developing and implementing individualized care plans, and collaborating with the interdisciplinary team to ensure seamless care transitions.
  • Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
  • Document patient information, including medical histories, test results, and treatment plans, in a timely and accurate manner.
  • Participate in the development and implementation of individualized care plans, and collaborate with the interdisciplinary team to ensure seamless care transitions.
  • Communicate effectively with patients, families, and healthcare team members to ensure that patients' needs are met and that care is delivered in a timely and efficient manner.
Requirements:
  • Current State Nursing License
  • Minimum 1-year acute care experience in a hospital setting
  • Appropriate certifications for the specific position
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Excellent communication and interpersonal skills
