Responsibilities
  • Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans

  • Administer medication and treatment as prescribed by the physician, and provide education and support to patients and families
  • Document patient progress and outcomes accurately and thoroughly
  • Participate in ongoing education and professional development to stay current with best practices and industry developments
Requirements
  • Graduate of an accredited nursing program with current licensure to practice in the state of operation
  • At least one year of nursing experience, with three or more years preferred
  • Experience in home health or a similar setting is a plus
