Join Our Team as a Community Support Planner

Service Coordination, Inc. is seeking dedicated individuals to fill the role of Community Support Planners, also known as Case Managers. We are looking for passionate professionals who are eager to make a difference in their communities and provide exceptional support to those in need.

Key Responsibilities:

  • Provide targeted case management by advocating for clients, arranging necessary services, and monitoring their progress.
  • Build and maintain positive relationships with clients and stakeholders.
  • Facilitate the development of person-centered service plans and ensure they are regularly updated.
  • Document activities and maintain accurate records while handling sensitive information responsibly.
  • Identify and secure ongoing support services, assisting clients in navigating available resources.
  • Conduct regular follow-ups to monitor the effectiveness of services and make necessary adjustments.

Qualifications:

  • A Bachelor’s or Master’s degree in a relevant field such as Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
  • A valid driver’s license and access to an insured vehicle.
  • Experience working with individuals with disabilities is preferred.
  • Familiarity with person-centered planning is a plus.
  • Willingness to undergo a background check.
