Position Overview:

As a Support Planner, you will engage in targeted case management, advocating for, coordinating, and monitoring services for individuals utilizing Medicaid Home and Community-Based services across various age groups and medical needs.

Key Responsibilities:

  • Build and maintain positive relationships with clients and stakeholders.
  • Facilitate the development of person-centered service plans and monitor their implementation throughout the year.
  • Document activities promptly and maintain accurate records while handling sensitive information.
  • Identify and secure ongoing support services tailored to individual needs.
  • Conduct regular follow-ups to ensure service quality and compliance with state standards.

Qualifications:

Applicants should possess a Bachelor's or Master's degree in a relevant field such as:

  • Human Services Administration
  • Psychology
  • Sociology
  • Social Work
  • Nursing
  • Counseling

Experience working with individuals with disabilities and familiarity with person-centered planning is preferred. Candidates must also have a valid driver's license and an insured vehicle.
