We are seeking a skilled Social Worker II to join our Ambulatory Care team at Hennepin County Medical Center. As a key member of our multidisciplinary team, you will provide direct service to clients and their families, conducting social work assessments, interventions, and treatment plans. Your expertise will help address the psychosocial aspects of healthcare, ensuring our patients receive comprehensive care.

Key Responsibilities
  • Conduct high-risk screenings and referrals to identify patients requiring social work assessment and intervention.
  • Perform bio-psychosocial assessments to determine problems and develop appropriate service/treatment plans.
  • Collaborate with the interdisciplinary team, community, and healthcare resources to implement care plans.
  • Provide supportive counseling, crisis intervention, and care coordination, including discharge/transition planning.
  • Advocate for patients and families, ensuring their rights and needs are met.
Requirements
  • Master's degree in social work or a related field.
  • Unrestricted license by the Minnesota Board of Social Work at one of the following levels: LGSW, LISW, or LICSW.
  • Experience with electronic health records and software programs.
  • Demonstrable cultural competence and ability to interact effectively with diverse populations.
