Social Worker II

We are seeking a highly skilled Clinical Social Worker to join our Ambulatory team at Hennepin County Medical Center. As a key member of our care coordination team, you will provide direct service to clients and their families, including social work assessment, interventions, and treatment plans.
Responsibilities- Identify patients who require social work assessment and intervention through high-risk screening and referrals.
- Interview clients and perform bio-psychosocial assessments to determine problems and appropriate types and methods of service/treatment.
- Design and follow through on intensive long or short-term service/treatment/care plans, collaborating and communicating with the interdisciplinary team, community, and healthcare resources.
- Provide supportive counseling and crisis intervention using social work methods under the supervision of a licensed independent clinical social worker (LICSW).
- Interpret and explain policies, regulations, and programs to clients and the public.
- Refer clients to appropriate community services, provide support and assistance in connecting to services as needed, and interpret social and emotional factors to others involved in treatment of clients.
- Advocate for patients and families by supporting patient rights, ethical decision making, and accessing protective services.
- Master's degree in social work or a related discipline.
- Unrestricted license by the Minnesota Board of Social Work at one of the following levels: LGSW, LISW, or LICSW.
- Ability to counsel and instruct patients and families.
- Ability to communicate with patients and families under sometimes stressful circumstances.
- Ability to recognize and document patient progress.
- Ability to work cooperatively with other members of the multidisciplinary clinical team.
