Job Summary

We are seeking a highly skilled Registered Nurse II to join our Hospice Unit at Northern Light Home Care and Hospice. As a Registered Nurse II, you will be responsible for providing high-quality patient care to our patients and their families. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.

Responsibilities
  • Patient Safety: Maintain a safe and therapeutic environment by understanding and complying with agency policies and procedures, and identifying and communicating potential safety issues.
  • Clinical Assessments: Perform and document comprehensive and holistic assessments, interpret and respond to abnormal diagnostic data, and anticipate potential emergency situations.
  • Caring and Code of Conduct: Develop a relationship with patients and families that facilitates mutual involvement in the planning of care, treat patients with courtesy and respect, and seek resources for reconciling personal issues and biases.
  • Critical Thinking: Demonstrate basic critical thinking skills.
  • Documentation: Document patient care in a concise, accurate, and complete manner, reflecting adherence to agency, regulatory, and legal requirements.
  • Population-Specific Competency: Complete population-specific mandatory education competencies, identify age, cultural, or other population-specific patient needs, and work collaboratively with patients and families of diverse backgrounds to develop care plans.
  • Evidenced-Based Practice and Research: Practice is consistent with Northern Light Health policy and procedures, understand evidence for practice change, and demonstrate knowledge of reliable resources to support evidenced-based practice.
  • Leadership and Professional Conduct: Utilize effective time management, meet expectations for punctuality and attendance, demonstrate flexibility in scheduling, and contribute to a healthy work environment by consistently utilizing effective communications with the interdisciplinary team.
  • Collaboration and Teamwork: Actively implement and update multidisciplinary care plans, collaborate with internal and external resources to meet patient care needs, and contribute to a healthy work environment by consistently exhibiting professional behaviors and addressing interpersonal conflicts.
  • Resource Management: Identify opportunities to improve utilization of resources.
  • Professional Development: Take responsibility for meeting education and mandatory programs, complete clinical evidence-based competencies, and seek opportunities to promote and advance professional growth.
  • Performance Improvement: Demonstrate knowledge of and contribute positively to department performance improvement outcomes, identify potential safety issues, and report to department manager and/or local or agency-based practice committee.
  • Productivity: Provide service and produce work at or above expected levels of quality and consistent with agency-defined quantitative standards.
Requirements
  • Credentials: Required Registered Nurse
  • Education: Required Associate's Degree, Bachelor's Degree Preferred
  • Experience: 1 year/years of Clinical Experience
Working Conditions
