RN - Hospice Care Specialist

We are seeking a skilled and compassionate RN Nurse Clinician to join our Hospice team. As a member of our team, you will provide holistic care to terminally ill pediatric and adult patients and their families, assessing their needs and developing personalized plans of care.
Key Responsibilities- Assess patient needs and develop comprehensive plans of care
- Provide holistic care to patients and their families, addressing physical, emotional, and spiritual needs
- Collaborate with interdisciplinary teams to coordinate care and services
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care
- Document patient progress and outcomes in accordance with regulatory requirements
- Associate's or Bachelor's degree in Nursing
- Washington Registered Nurse License
- 1 year of nursing experience, with some pediatric nursing experience required for pediatric programs
- Prior experience in hospice, homecare, long-term care, medical/surgical, and/or oncology settings
