Job Title: Licensed Practical Nurse - Geriatric Care

We are seeking a skilled Licensed Practical Nurse to join our team at Trinity Health PACE in W. Springfield, MA. As a LPN, you will provide high-quality care to our elderly participants, promoting their independence and well-being.

Job Summary:

The primary purpose of this position is to provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders. You will work collaboratively with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each participant.

Responsibilities:
  • Provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders.
  • Develop and implement comprehensive care plans that meet the unique needs of each participant.
  • Communicate effectively with participants, families, and healthcare providers to ensure seamless care coordination.
  • Participate in daily program activities and provide education and support to participants and families.
  • Assist with medication management and documentation.
Requirements:
  • Graduate of an accredited Nursing Degree program and current, unencumbered, active license to practice as a Licensed Practical Nurse in the state of Massachusetts.
  • Active CPR Certification required.
  • Minimum of two (2) years of demonstrated successful experience in related services with geriatric population.
  • Ability to perform standard clinical procedures according to established protocols and a high level of accuracy and thoroughness in documenting the delivery of services.
  • Superior written and verbal communication skills appropriate for audience comprehension.
